Citation for Distinguished Service Medal: PERKINS, Elwyn, Petty Officer, 3449, Distinguished Service Medal, RCN, HMCS ST LAURENT. Awarded as per Canada Gazette of 9 September 1944 and London Gazette of 15 August 1944. "For leadership, skill and determination in H.M. Ships Affleck, Brecon and Forester, and H.M. Canadian Ships St. Laurent, Swanse aand Owen Sound in operations against U-Boats. For good service in the destruction of a submarine on 10 March 1944."
Chief Perkins joined the RCN 31 Jul 1939 and retired 30 Jul 1960
